Incident reports will tell you that an incident occurred at a particular address and this will allow you to pull a permit to have repairs completed for or insurance company purposes.

Fire and ambulance incident reports are available to the public for a fee of $2.50 each. You may request a report by completing the form below. Please mail your check to:

Ambulance reports require that the patient sign a release form. If you are requesting an ambulance report, please click below to open a printable copy of the release. The release must be printed, filled out, signed and mailed in with your check. If you cannot view or print the release form, please fill out the mailing address section of the form below, check the box marked "mail a release form," and we will be happy to send you one.
