Reporting Incidents

To report an incident to Risk Management for investigation, fill out the Citizen Incident Report Form and send it, along with any other important information relative to the incident, to Risk Management at 3430 Court House Drive, Ellicott City, MD 21043.  It can also be emailed to risk@howardcountymd.gov or faxed to (410) 313-6399

NOTE: The Citizen Incident Report Form is for use only when damage or injury involves Howard County property, vehicles, or is caused by Howard County employees. See the Frequently Asked Questions section below for more information regarding this form.
