About
This workgroup focuses on improving outcomes for individuals with diagnosable chronic health conditions through coordinated and integrated care. Its scope includes evaluating the availability and effectiveness of screenings for hypertension, diabetes, asthma, and mental health, as well as reviewing the integration of physical and behavioral health services. Based on this review, the workgroup will develop recommendations for the Howard County Executive to strengthen chronic disease prevention and management across the county.

Responsibilities
This workgroup reviews countywide efforts related to chronic disease screening, prevention, and management. Members assess the effectiveness of existing services, the integration of physical and behavioral health care, and opportunities to strengthen coordinated care models that improve long-term health outcomes.
