The Community Living Program (CLP) conducts a comprehensive in-home assessment by an Occupational Therapy practitioner to evaluate the environment for safety risks and decreased function (as needed). Due to COVID these in home assessments are conducted virtually.
The therapist works with a person to prioritize areas of need and appropriate solutions. Frequent interventions involve development of emergency response plans, removal of environmental hazards, training on adaptive equipment, and education in adaptive or compensatory strategies during daily activities. Recommendations maybe made for durable medical equipment and/or adaptive equipment to improve safety and independence in the home. Referrals may be made to other Office on Aging and Independence programs.
