Requesting Incident Reports

Fire and ambulance incident reports are available to the public for a fee of $2.50 each. You may request a report by completing the form below. Please mail your check to:

Checks should be made out to "Director of Finance, Howard County."

Ambulance reports require that the patient sign a release form. If you are requesting an ambulance report, please click here to open a printable copy of the release. The release must be printed, filled out, signed and mailed in with your check. If you cannot view or print the release form, please fill out the mailing address section of the form below, check the box marked "mail a release form," and we will be happy to send you one.
